This form is used to list all of the graduating student's completed high school courses and corresponding grades. The course options on this form are from our course flyer, which can be viewed at our Graduation Requirements page. Occasionally, a course that the student has taken might not perfectly match up with the titles of our listed courses. In that case, we recommend selecting the course that appears the closest in content to the course the student has taken, as course names vary by curriculum. As always, please feel free to contact us at with any questions you have during this process!

As a reminder, the cost for our graduation service is $95, and you will be forwarded to PayPal to submit your payment upon the submission of your Graduation Application.
